Case 20: Follicular Keratosis
Our study in on patients with in their early 50s , in adulthood, having warty growth of Follicular Keratosis. It is a pre cancerous state where lumps of swollen underneath skin segments starts spreading across different part of body on the outer skin in faces, back of the body, front of the body, legs, hands, near eye brow just above eye opening or being in the ear or just underneath the ear on the neck. It can spread while shaving across face. Symptoms come with slightly painful infection where the lumps grow. it can grown vertically under the skin layer or outer skin horizontally.
Signs & Symptoms observed
Follicular Keratosis is a inherited disease where the severity of the disorder and the specific symptoms vary, differed among patients even among individuals within the same family origin. Keratosis follicularis, is also known as Darier disease, it is a genetic skin disorder an autosomal dominant trait , that is the offspring only needs one gene from one parent to have the trait or condition, a hereditary disease; it is not a Autosomal recessive disease, where the trait or condition will only manifest if a patient inherits it from “both of parents.”
The affected patients developed skin lesions that consist of thickened, rough bumps (papules) or plaques and have a brown or yellow crust. These hardened, scaly lesions progressively grew over time over 4-5 months as understood and gradually grew bigger or spread. The mucous membranes (made from ectodermal tissue) are also affected in most patients.
Several patients had periods of time when signs improved as if there was a period of remission, but the lesions recurred or relapsed again. Keratosis follicularis is inherited as an autosomal dominant trait.
The initial lesions in keratosis follicularis were small, firm, bumps (papules) that were skin-colored, brown in color. The lesions usually affect the areas of the body near sebaceous glands (sebaceous glands secrete oily grease) including the chest, back, forehead and some spotsin the scalp. Darier disease may affected skin creases skin folds, e.g. groin.
The skin lesions associated with keratosis follicularis developed a brown, greasy crust and become thickened and warty (hyperkeratotic). The lesions became warty plaques that covered extensive areas of the body, the entire body was becoming affected. The lesions caused persistent itchiness (pruritus). Some patients skin becomes raw (erosions) and painful.

Skin also developed a combination of bacterial, viral or fungal infections (secondary infections) that had worsen (exacerbate) the initial condition. Infected skin lesions gave off a distinct, unpleasant (malodorous) smell.
Patients had been test " negative " for , herpes simplex virus (HSV) 1, 2 , which may be prone to infecting the lesions and causes pain in general cases in developing country where patients reside.
Patients were advised to avoid heat, exercise and sunlight which may also worsen keratosis follicularis or cause a new outbreak of lesions in their body. Patients complained that in winter it was more painful, had felt as if water was dropping in the ear and felt the lesions tend to recur (relapse) in winter than summer as investigated.
Rx -Prescription:
F Keratosis seems to appear on all patients in their adult hood between 45-50
EC electrodessication can treat as well as cryotherapy or cryo surgery. It can re occur and spread.
To control spread sealer cream and facid cream 2:1 ratio can be applied twice a day in the infected surroundings.
Sensitelial ultra rich dermatological bar soup can be used to wash with shower.
hypo-A lotion can be pre escribed to apply over the body after shower.
Tablet Cafaclav 500 ml (Ceruroxime ) can be used twice a day for seven days
Vitamin E USP Liqu -E twice a day , 200m, 14 days
With Vitamin A Retinol fort 50000 I U 2 tab twice a day for 14 days
Inverted Follicular Keratosis
It is noted to be associated with HPV 3, 11 along. Affected areas are back of ears, under chin, face, legs, face, forehead. Skin develops think lesions that spread both horizontally and vertically under the skin and affects mucus membrane to cause hair loss. Cryo therapy applied to infected areas. It is considered to be more severe because of tumor prone nature of it which spreads.
Histological anlaysis Reuqired.
